Please fill out as much as you can or are comfortable with. We have limited availability and will choose as many people as possible. The more we know about you and your story, the easier it will be for us to select you. 

We could invest thousands of dollars in ads on facebook, instagram, tic tok, etc. but that would only give money to already wealthy corporations – we would rather invest in our community and do something that we already love doing. 

There is no contract required. You’re more than welcome to use our free services and go straight back to your home gym or therapy clinic. If you choose to stay with us, great. If not, we hope that someone you know will get inspired by your story and think of us when they’re deciding where to go for their health and fitness.
